Assessment of the results of student conduct for Semester 1 of the 2023 – 2024 academic year (For students eligible for graduation in January 2024);
Summarizing the overall results of student conduct (Form 01) for students who will graduate in January 2024.
– The number of semesters used to summarize the assessment of overall student conduct is the number of semesters the student has participated in studying at the university. For early graduates of K59, only 7 semesters need to be filled in.
– The conduct assessment score for the semester in which the graduation courses are taken will be the score for participating in the Assessment of Student Conduct Results for Semester 1 of the 2023-24 academic year, or it will be taken from the score of the nearest previous semester when the student has a REQUEST FOR EXEMPTION from the Assessment of Student Conduct Results.
– Deadline for submitting the soft copy: Tuesday, January 2, 2024
– Deadline for submitting the hard copy: Thursday, January 4, 2024
– Students must accurately record their contact phone number in the requests for exemption from the Assessment of – Student Conduct Results or in the Student Conduct Assessment Form. The faculty will check the soft copy first. If the student does not receive feedback from the faculty (024.3259.5158) before 12pm on Wednesday, January 3, 2024, the student’s soft copy is accurate and the student can print out the hard copy to submit to the faculty office B211.

– K59 students create individual folders in the administrative class folders at this link
– Other students create individual folders, named: Khoa-Họ tên-Mã sinh viên (Class-Full Name-Student ID) and – upload the soft copy to this folder
– Required documents:
1. Summary of Overall Conduct Assessment (Form 01), note that the final semester score is the score on the exemption request.
(If the student has more assessment semesters than the available spaces on the form, add columns and calculate the average of all semesters)
2. PDF file of the transcript on ftugate (showing the full conduct assessment scores for each semester)
3. Request for exemption from the Conduct Assessment for Semester 1 of the 2023-24 academic year or the Conduct Assessment Form for Semester 1 of the 2023-24 academic year
For students who are missing conduct assessment scores in previous semesters, the student needs to do the following additional:
4. Request for supplementary conduct assessment for the missing semesters
5. Supplementary self-assessment form for conduct: The activities that the student participated in the supplementary semester (section 1.2, section 3.2 and section 5) will not be counted towards the conduct assessment score.
